Do you have evening or weekend appointments?
Appointments are available afternoons and early evenings, not on weekends.
Are there forms I need to fill out before the first appointment?
You can print and fill out the New Client Information form (see link below) and scan or send a picture of it to me via text or email.
Payments
Do you take insurance?
I am not an "in network" provider with any insurance but if your plan has "out of network" coverage, they will reimburse you for part of the fee.Here are questions to ask your insurance company to determine your coverage:
- Does my plan include a mental health benefit?
- Does my plan cover out-of-network therapists?
- If yes, do I have a separate mental health deductible?
- How much is it per calendar year?
- How much have I met so far?
- Do I need pre-approval or pre-certification before I start sessions?
- If yes, how do I get pre-certification?
- What percentage of the usual and customary fee does plan pay per session?
- What percentage of fee is my copay?
- Is there a limit on number of sessions?
- Do you cover CPT 90837 or only CPT 90834?
- Where do I mail claims to get reimbursed?
- What is your Payor ID for electronic submission of claims?
- Once I send in a claim, how long before I receive reimbursement?
How do I pay for appointment? Do I pay every session or do you bill me?
Appointments are paid at the time of each appointment. I accept all credit, debit, HSA/FSA cards through Zelle or Apple Pay.
